USFA LODD: New York Firefighter On-Duty Death Notification 12.18.2019

On Wednesday, December 18, 2019, at approximately 1140hrs., Firefighter Michael Cardinale, Sr., 63, responded to a vehicle crash.  After returning to the station, he went upstairs to the sitting area.  Approximately 25 minutes later, a fellow firefighter found him unresponsive and immediately performed life-saving efforts to revive him.  Firefighter Michael Cardinale, Sr. was transported to the hospital where he was pronounced deceased.

USFA LODD: Pennsylvania Firefighter On-Duty Death Notification 12.03.2019

On Monday, December 2, 2019, at approximately 2000hrs., Captain Michael Christopher Malinowski Sr., 40, responded to a call of reported downed wires and trees.  After returning to the station, he complained to his fellow firefighters that he wasn’t feeling well.  The next day, Tuesday, December 3, 2019, while at work, Captain Michael Christopher Malinowski Sr. became ill and was immediately taken to the hospital where he passed away a short time later from an apparent heart attack.

USFA LODD: Colorado Firefighter On-Duty Death Notification 12.07.2019

On Saturday, December 7, 2019, at approximately 0200hrs., Firefighter Kenneth Jones, 46, responded to a multi-residential structure fire.  Firefighter Jones had climbed to the roof of the five-story building in search of access to the fire when he fell off of the structure.  Firefighter Kenneth Jones was immediately transported to the hospital where he passed away.

USFA LODD: Georgia Firefighter On-Duty Death Notification 12.05.2019

On Wednesday, December 4, 2019, at approximately 1545hrs., Firefighter George “Johnny” Childs, 67, responded to a residential structure fire.  The next morning, Thursday, December 5, 2019, he was found unresponsive by his personal vehicle outside a local business.  He was immediately taken to the hospital where he passed away from cardiac arrest.

On Tuesday, November 19, 2019, at approximately 1900hrs., Fire Chief Darryl Rollins, 42, responded to a possible residential fire.  He returned home and was found unresponsive early the next morning.  He was immediately taken to the hospital where he passed away from a cardiac arrest.

Massachusetts Firefighter On-Duty Death Notification 11.13.2019


In the early morning hours of Wednesday, November 13, 2019, Lieutenant Jason Menard, 39, and his crew responded to a multi-residential structure fire, with heavy smoke coming from the building. The fire grew to four alarms and, at one point, firefighters issued a mayday call from the third floor.  Lieutenant Menard and other firefighters became trapped by heavy fire conditions.  He helped two firefighters escape the blaze, but the fire quickly worsened and Lieutenant Menard was unable to flee and succumbed to his injuries. Three other firefighters were taken to the hospital.  Two of the firefighters have been released and the third is in serious condition.

On Monday, November 11, 2019, at approximately 0200 hrs., Fire Chief John Sing, 48, responded to an EMS call.  When he returned to the station, he collapsed and was immediately taken to the hospital where he passed away from an apparent heart attack.

USFA LODD: Florida Firefighter On-Duty Death Notification 11.06.2019

On Wednesday, November 6, 2019, at approximately 0350hrs, Assistant District Chief Dwain S. Bradshaw, 41, of the Beulah Fire Station of Escambia County Fire Rescue responded to a fatal motorcycle accident.  While at the scene, he was struck by an 18-wheeler log truck.  Chief Bradshaw was immediately airlifted to the Sacred Heart Hospital in Pensacola, Florida, where he passed away.  Chief Bradshaw also served as the District Chief of the Escambia County Fire Rescue Station #1 in the community of Bellview.

USFA LODD: Wisconsin Firefighter On-Duty Death Notification 11.02.2019


On October 19, 2019, Firefighter Brian Serdynski, 38, suffered a heart attack while at the scene of a motor vehicle accident. He was transported to a hospital where he was treated and then released on October 19, 2019. In the very early morning hours on November 02, 2019, Firefighter Serdynski was having difficulty breathing. First responders from Union Grove-Yorkville and Kansasville fire departments responded to Firefighter Serdynski’s home and immediately administered medical care. Despite their efforts, Firefighter Brian Serdynski was pronounced deceased at his home.

USFA LODD: Washington, DC Firefighter On-Duty Death Notification 10.27.2019


On Thursday, October 24, 2019, Firefighter Alex Graham, 48, was conducting pump testing at the D.C. Fire and EMS Training Academy, when he suffered a medical emergency.  He was immediately taken to the hospital, where he passed away on Sunday, October 27, 2019, from an aortic dissection.

USFA LODD: Alabama Firefighter On-Duty Death Notification 10.18.2019

On October 18, 2019, Firefighter Michael Johnston, 45, was driving the Equality Volunteer Fire Department’s 2004 GMC water tender when it overturned while responding to a brush fire. Firefighter Johnston was ejected from the truck in the single-vehicle accident and pronounced deceased at the scene from the injuries he sustained.

On October 19, 2019, Firefighter Robert Bush fell, 49, ill during fire department training. Other firefighter department members immediately provided medical assistance to Firefighter Bush and then transported him to a local hospital. Despite revival efforts, Firefighter Bush passed away at the hospital. The nature and cause of fatal injury are still to be determined.

USFA LODD: Texas Firefighter On-Duty Death Notification 10.12.2019

On Monday, October 7, 2019, Firefighter Steven Paul Henderson, 60, and another firefighter were returning from a fire call in a tanker truck when they stopped alongside the road to inspect the front tires. While outside the vehicle, another firefighter from the same fire department driving his personal pick-up truck, who was also at the scene of the fire call and left shortly after Firefighter Henderson, did not see the truck parked in the roadway or the two firefighters standing outside of it and subsequently hit both Henderson and the other firefighter. Firefighter Henderson was immediately transported by LifeFlight to the Memorial Hermann-Texas Medical Center, in Houston, where, on Saturday, October 12, 2019, he succumbed to the injuries sustained in the accident.  The other firefighter who was hit had non-life threatening injuries and was released from OakBend Medical Center in Richmond, TX..

USFA LODD: Texas Firefighter On-Duty Death Notification 10.15.2019

On October 15, 2019, Firefighter Greg Garza, 49, responded to an electrical incident at the Comfort Suites.  Firefighter Garza returned to the apparatus to retrieve some equipment and was attempting to exit the fire apparatus when he tripped getting out of the truck and was hit by an oncoming vehicle.  He went into traumatic arrest, was transported and passed away at the hospital.

USFA LODD: Washington Firefighter On-Duty Death Notification 10.02.2019


On Sunday, September 1, 2019, the Okanogan Fire Department/Okanogan County Fire District 3 responded to the Spring Coulee wildland fire.  Assistant Chief Christian Dean Johnson, 55, and another firefighter were in a brush truck on the north end of the fire when the flames overtook them.  The firefighter with Assistant Chief Johnson was able to outrun the fire, but Johnson was unable to escape.  The other firefighter came back to treat him and called for help.  Assistant Chief Johnson was airlifted to the Harborview Medical Center in Seattle, WA, where he died on Wednesday, October 2, 2019, from the burn injuries he sustained in the fire.

USFA LODD: Calfornia Firefighter On-Duty Death Notification 10.02.2019

On Wednesday, October 2, 2019, Captain Paul Rotondaro, 36, while on-duty, left the Merced County fire station to go to the Usona fire station.  Driving a Ford Ranger, Captain Paul Rotondaro was hit by another truck, causing his truck to leave the road and catch fire, killing him instantly.

USFA LODD: South Carolina Firefighter On-Duty Death Notification 10.04.2019

Fire Engineer Paul Quattlebaum, 46, was dispatched to a medical call. While responding to the call, he and his partner saw a vehicle accident in the 5200 block of Fairview Road and stopped to help. While checking on those involved, Fire Engineer Quattlebaum was struck by a semi-truck. EMS treated Fire Engineer Quattlebaum at the scene and then transported him to a nearby hospital where he passed away.

On September 26, 2019 after returning home from his shift at the Martin County Fire Rescue Department, Station 16, in which he participated in physical fitness training, Firefighter-Paramedic Scott R. Neumann, 52, passed away from a medical incident which has not yet been determined.

On September 21,2019 while on duty at Houston Fire Department’s Station 27, Firefighter Kenneth Stavinoha, 36, suffered a medical emergency and did not survive. The cause of death is pending an autopsy.

On September 21, 2018, Firefighter Edward Nulton, 60, was directing traffic at the scene of a vehicle accident when he was struck by a dump truck. He suffered critical injuries, including traumatic brain injury, from which he was unable to recover. Nulton passed away as a result of his injuries on September 19, 2019.

On September 20, 2019, While operating a fire department vehicle, Captain Claud Messer, 74, was involved in a vehicle accident on Jonathan Creek Road (Highway 276 North) as he responded to another emergency traffic accident call. Captain Claud Messer passed away as a result of the injuries sustained in the accident.

USFA LODD: Maine Firefighter On-Duty Death Notification 09.16.2019

On September 16, 2019 employees inside of a recently renovated two-story administrative office for Life Enrichment Advancing People (LEAP) Inc., a nonprofit that helps adults with developmental, cognitive, and intellectual disabilities, made an emergency call reporting that they smelled propane. No patients were inside and all employees inside of the building evacuated immediately. Captain Michael Bell, 68, responded with the Farmington Fire Rescue to the emergency call. Soon after firefighters arrived on scene and began to investigate, there was an explosion that leveled the building. Captain Bell was killed as a result of injuries sustained in the explosion. Six other firefighters, including the fire chief, and a maintenance worker were reportedly injured and transported to hospitals around the state.

USFA LODD: New York Firefighter On-Duty Death Notification 08.10.2019

On Friday, August 9, 2019, Lieutenant Brian J. Sullivan, 54, worked a 24-hour shift in which he responded to seven emergency calls.  During his shift, Lieutenant Sullivan complained to others of having pain and discomfort but continued to respond to calls.  When he returned home after his tour of duty, he suffered a heart attack and was immediately transported to the Orange County Regional Medical Center where he passed away in the early morning of Saturday, August 10, 2019.

On Friday, August 2, 2019, Assistant Chief Walter Wagaman, 46, was responding to an emergency as a passenger in an apparatus when he was ejected from it.  Assistant Wagaman was treated by emergency personnel at the scene and was immediately flown to the York Trauma Center where, on Wednesday, August 14, 2019, he passed away from the injuries he sustained from the accident.

On Saturday, August 10, 2019, Firefighter Norman Edward Klenow, 78, responded to a motor vehicle accident.  While providing traffic control at the scene, he complained of not feeling well and was assisted into the apparatus.  After returning to the fire station, he went home, when at approximately 1946hrs, was found in cardiac arrest.  Firefighter Norman Edward Klenow was immediately transported to the hospital where he passed away.

USFA LODD: Florida Firefighter On-Duty Death Notification 08.06.2019

On the evening of August 6, 2019, Fire Chief/Public Safety Director James E. Sommers, 40, was returning home on his motorcycle from a Suwannee County Commission Meeting when he was struck by a vehicle. Fire Chief/Public Safety Director Sommers was immediately transported to the Lake City Medical Center where he died from the injuries he sustained in the accident.

USFA LODD: California Firefighter On-Duty Death Notification 07.29.2019

On Sunday, July 28, 2019, Firefighter Yaroslav Katkov, 29, was on a training hike with his crew in San Diego County.  During the hike, he suffered a medical emergency and was immediately flown to Temecula Valley Hospital in Riverside County where he passed away on Monday, July 29, 2019.

USFA LODD: New Mexico Firefighter On-Duty Death Notification 07.21.2019

On June 5, 2019, the City of Roswell Fire Department was preparing for the community July 4th fireworks show.  Driver/Operator Jeff Stroble, 46, was assisting with this preparation when an explosion occurred, injuring him.  Driver/Operator Stroble was immediately taken to the hospital but passed away on July 21, 2019, from the injuries he sustained in the explosion.

USFA LODD: Pennsylvania Firefighter On-Duty Death Notification 07.16.2019

On July 16, 2019, The North Belle Vernon Volunteer Fire Department was dispatched at 0132hrs to assist on a mutual aid residential fire. Fire Lieutenant Neil B. Cope, 46, was part of the engine crew assigned to staging in full PPE and SCBA with masks doffed. After approximately 45 minutes the crew was released by command and cleared the scene. After returning to his residence, Lieutenant Cope spoke to his wife at 0730hrs via phone. His wife then attempted to call him multiple times during the day without success.  At approximately 1600hrs his wife came home and found him not breathing and cold to the touch on the floor of the bedroom. EMS was called and Fire Lieutenant Neil B. Cope was pronounced deceased at home.  The cause and nature of death are under investigation.

USFA LODD: South Carolina Firefighter On-Duty Death Notification 05.22.2019

On May 22, 2019, Fort Jackson Environmental Division personnel initiated a 424 acre prescribed fire operation for wildlife habitat improvements and fuel management. During the prescribed burn, wildlife biologist and certified Wildland Firefighter Angela "Nicole" Chadwick-Hawkins, 45, was assigned a ground ignition operation, utilizing an All-Terrain-Vehicle (ATV) to conduct the task. At some point while conducting her assigned duties, personnel lost radio contact with Wildland Firefighter Hawkins. A search of her last known location was conducted during which Wildland Firefighter Hawkins’ body was found. At approximately 1230hrs, Fort Jackson Fire Department units were dispatched along with Moncrief Army EMS and Fort Jackson Military Police. Upon arrival, emergency personnel determined that Wildland Firefighter Hawkins sustained fatal injuries and was declared deceased by medical personnel. Additional details are pending the completion of investigative processes by the Army Criminal Investigation Division with support and assistance from the Bureau of Alcohol, Tobacco, Firearms, and Explosives as well as the US Army Combat Readiness/Safety Center Accident Investigation Team.
