USFA LODD: Wisconsin Firefighter On-Duty Death Notification 04.25.2018

On April 24, 2018, at 2319hrs, Firefighter Philip H. Neubich, 69, responded to the fire station when his fire department was paged out for a gas leak inside of a residence. Several hours later, at approximately 0615hrs, at home, Firefighter Neubich was found unresponsive by his wife. Local EMS responded, but Firefighter Neubich was found to be deceased. The nature of fatal injury is reported to ave been a heart attack.

USFA LODD: California Firefighter On-Duty Death Notification 04.21.2018

Inmate Firefighter Trainee Anthony Colacino, 33, was participating in a training hike when he collapsed just before 0800hrs on April 21, 2018. Fellow inmate firefighters attempted to revive Firefighter Trainee Colacino before other emergency services personnel took over care. Despite all efforts, Firefighter Trainee Colacino succumbed to a nature and cause of fatal injury still to be determined.

USFA LODD: Virginia Firefighter On-Duty Death Notification 04.19.2018

Firefighter Herbert Tyler "Butch" Wilcox, 77, responded to two fire calls on April 18, 2018, one at 1300hrs and a brush fire at 1605hrs. After returning from the brush fire, Firefighter Wilcox complained of neck and back pain. He went to a local MediCenter where he was examined and then sent home. At approximately 0730hrs the next morning, April 19, 2018, Firefighter Wilcox's wife tried to wake him but found him unresponsive and immediately called 911. Emergency responders arrived on scene and began CPR, but after 30 minutes of resuscitation efforts, Firefighter Herbert Tyler "Butch" Wilcox was pronounced deceased at his residence. The nature and cause of fatal injury is still to be determined.

USFA LODD: Georgia Firefighter On-Duty Death Notification 03.30.2018

On March 30, 2018 late in the evening of his scheduled shift and after conducting search and rescue drills in full protective gear including SCBA, Firefighter Adam Taylor, 30, began to complain of chest pains he related to a muscle pull.  Shortly after leaving work to go home because of the pain, Firefighter Taylor went into cardiac arrest and was pronounced deceased upon arrival at a local hospital.

Our condolences to the Taylor Family & Friends, Decatur County Fire Rescue, and to all affected by Firefighter Adam Taylor’s passing.

Firefighter Gregory Jackson, 50,  became ill at the end of his shift on April 14, 2018.  Firefighter Jackson was transported to a local hospital where he passed away on April 19, 2018. The nature and cause of fatal injury are still to be reported.

USFA LODD: New York Firefighter On-Duty Death Notification 04.17.2018

Shortly after responding to numerous calls on the evening of Monday, April 16, 2018, Firefighter-Paramedic M. Kent Stillwagon, 52, was found just after midnight at his residence in cardiac arrest. His fellow volunteers and friends from the Greenlawn Fire Department responded and tried valiantly to resuscitate him, but after being transported to Huntington hospital and an extended resuscitation attempt by hospital staff, Firefighter-Paramedic Stillwagon passed away.

Our condolences to the Stillwagon Family & Friends, Greenlawn Fire Department, and to all affected by Firefighter-Paramedic M. Kent Stillwagon’s passing.

USFA LODD: North Carolina Firefighter On-Duty Death Notification 04.08.2018

On April 8, 2018 Fire Chief Richard L. Sales, 52, was killed in a single vehicle crash while traveling on fire department business. According to initial reports, Sales was on I-40 Eastbound when he struck barriers in the middle of the road, lost control of his vehicle, and went down an embankment onto roadways below before coming to a stop. The cause of the wreck is still under investigation, but the local news reported it was the same area where a motorcyclist and the driver of a semi-trailer died in separate wrecks on March 31, 2018.

USFA LODD: Texas Firefighter On-Duty Death Notification 04.05.2018

On April 5, 2018 Assistant Fire Chief Bernard Olive, 68, passed away while on duty from a nature and cause of fatal injury still to be determined.

USFA LODD: Texas Firefighter On-Duty Death Notification 04.03.2018

Firefighter Caleb Scott, 29, suffered a reported heart attack while at his fire station on the morning of April 3, 2018. Fellow responders immediately came to his aid at the firehouse before transporting him to Medical City North Hills Hospital, where Firefighter Scott later succumbed to his injury.

Second Assistant Fire Chief Timothy Wayne Royce, 58, died from an apparent heart attack on March 30, 2018, several hours after responding with his fire department to a motor vehicle crash and a medical call, as well as participating in fire department training at the station.

USFA LODD: Wisconsin Firefighter On-Duty Death Notification 03.09.2018

Firefighter-Paramedic Richard L. Garner Jr., 29, completed a 48 hour shift at 0700hrs on April 1, 2018, during which he responded to nineteen emergency calls.  Later that day, between the times of 1200hrs and 1300hrs, Garner collapsed at a private residence in Mount Pleasant, Wisconsin. Firefighter-Paramedic Garner was treated at the scene by the local EMS and transported to the hospital where he passed away from a nature and cause of fatal injury still to be determined.

On March 9, 2018 Driver Engineer Daryel Richards, 58, succumbed to a nature and cause of fatal injury still to be reported while on duty at the fire station on March 9, 2018.
