USFA LODD: Virginia Firefighter On-Duty Death Notification 09.23.2020

In the early morning hours of September 23, 2020, Firefighter/EMT Charles Woods, 58, responded to a residential fire in Marion, VA, with the Adwolfe Volunteer Fire Department. Upon arrival, the occupant of the residence was secured and fire suppression activities began. During operations, Firefighter/EMT Woods was found unresponsive in a tanker after having filled it from a nearby fire hydrant and completing several other on-scene duties. Crews at the scene immediately provided lifesaving efforts and transported Firefighter/EMT Woods to Smyth County Community Hospital. All attempts to revive Firefighter/EMT Woods, however, were not successful.

USFA LODD: Virginia Firefighter On-Duty Death Notification 10.11.2018

On Thursday, Oct. 11, 2018, at approximately 2100hrs, the Virginia State Police responded to a two-vehicle crash involving a fatality at southbound I-295, north of exit 38B.
    
The trooper’s preliminary investigation revealed that Hanover County Fire and EMS was on-scene providing assistance with a two-vehicle crash with all of its emergency equipment activated, when a tractor trailer traveling southbound struck the engine in the rear.
   
A total of three Fire and EMS personnel were injured in the crash with one of them, Lieutenant Brad Clark, 43, succumbing to his injuries at the scene. The other two were transported to the hospital with serious injuries sustained at the scene from the crash. The driver of the tractor trailer had to be extricated from the cab of the truck and was transported for treatment with serious injuries.

Firefighter William Moore IV, 48, suffered a fatal heart attack during a training exercise on June 14, 2018.

USFA LODD: Virginia Firefighter On-Duty Death Notification 04.19.2018

Firefighter Herbert Tyler "Butch" Wilcox, 77, responded to two fire calls on April 18, 2018, one at 1300hrs and a brush fire at 1605hrs. After returning from the brush fire, Firefighter Wilcox complained of neck and back pain. He went to a local MediCenter where he was examined and then sent home. At approximately 0730hrs the next morning, April 19, 2018, Firefighter Wilcox's wife tried to wake him but found him unresponsive and immediately called 911. Emergency responders arrived on scene and began CPR, but after 30 minutes of resuscitation efforts, Firefighter Herbert Tyler "Butch" Wilcox was pronounced deceased at his residence. The nature and cause of fatal injury is still to be determined.

USFA LODD: Virginia Firefighter On-Duty Death Notification 05.19.2017

On May 19, 2017  Firefighter Roger Dale Johns, 63, was struck and killed by a backing piece of fire apparatus at the scene of a motor vehicle crash. Initial reports indicate that Firefighter Johns was acting as a ground-guide while the apparatus was backing up to block the roadway and protect the scene. Investigation into the fatal accident continues by authorities.

USFA LODD: Virginia Firefighter On-Duty Death Notification 06.06.2015

Veteran Fire Corps Crewmember Ian Haxton, 31, on June 6, 2015 at 0800hrs, suffered a medical emergency and collapsed 200 yards from the finish line while participating in the Wildland Firefighter Work Capacity Test. Medical care was immediately rendered by local emergency medical responders who had been staged on-site for the test. Crewmember Haxton was transported to an Advanced Care Facility where he passed away from a nature and cause of injury still to be determined. At the time of the fatal incident, Haxton was serving at the Buenos Aires National Wildlife Refuge in Sasabe, AZ, preparing for a series of prescribed burns scheduled to take place at the Refuge.

USFA: Virginia Firefighter LODD: On-Duty Death 12.14.2013

On December 14, 2013 at 1444hrs., while responding to the fire station for an alarm, Firefighter Joshua Smith, 25, was killed from injuries sustained when the privately owned vehicle he was operating left the roadway and crashed. Investigation of the fatal accident continues by local and state authorities.

Virginia Firefighter LODD, Died After Tanker Apparatus Crashes

Frederick County Fire and Rescue Officials informed during an afternoon press conference that 21-year-old Zachary Whitacre, a member of the Gore Volunteer Fire and Rescue Department , died after being taken to Winchester Medical Center.

Firefighter Whitacre died early this Monday morning in a single vehicle accident crash just over the West Virgina line for a mutual response for wate at around 4am this morningr. The tanker involved in the accident was heading to Capon Bridge to pick up water before responding to a reported structure fire in Hampshire County.

It was reported that Zachary Whitacre, who was riding in the back of a 1986 Ford fire tanker, was thrown out of the tanker and died afer the vehicle lost control on an icy Route 50, sending it into an embankment.

Donald Whitacre the driver of the tanker, and also Zachary's father, was taken to the hospital for non life-threatening injuries.

Fire Chief Linaburg described the tragity as one of the saddest moments in Frederick County's history.

The Gore Volunteer Fire & Rescue Department is one of eleven stations within the Frederick County Fire & Rescue Department. Gore is located about 10 miles from Winchester near the Virginia/West Virginia State Line.

Thoughts and prayers go out to Zach Whitacre’s Family, the Gore Volunteer Fire and Rescue Department, Frederick County Fire & Rescue Department, and Zach’s Friends.

Arlington VA Paramedic Joshua Weissman LODD

Official statement from the City of Alexandria:

It is with great sadness that Alexandria Fire Chief Adam K. Thiel announced the death of Paramedic Joshua Weissman at 4:25pm on Thursday, February 9.

While responding to a car fire on Interstate 395 near Glebe Road on the evening of February 8, Paramedic Weissman fell from the roadway, where he suffered a severe head injury. He was rescued by City of Alexandria and Arlington County Firefighters and Paramedics, with assistance from the City of Alexandria and Arlington County Police Departments and the Virginia State Police. Despite valiant efforts by the medical team at the Washington Hospital Center, Paramedic Weissman’s injuries proved to be fatal.

Joshua Weissman, 33, of Bristow, VA. was a seven-year veteran of the Alexandria Fire Department, and was hired in April 2006.
