USFA LODD: Texas Firefighter On-Duty Death Notification 03.23.2018

On March 23, 2018 the Caldwell County Office of Emergency Management has reported the death of Firefighter Larry Marusik, 68. Firefighter Marusik passed away at the Brooke Army Medical Center on March 23, 2018, from injuries sustained while battling a large grass fire on March 10, 2018.

Our condolences to the Marusik Family & Friends, Madison Ellinger Volunteer Fire Department, and to all affected by Firefighter Larry Marusik’s passing.

USFA LODD: Two West Virginia Firefighter On-Duty Death Notification 03.24.2018

On March 24, 2018 Assistant Fire Chief Michael Edwards, 46, and Lieutenant Tom Craigo, 40, were killed and three other firefighters were injured when, for a cause still under investigation, the apparatus they were in left the narrow roadway and crashed into a rock wall. At the time, the firefighters were responding to a multiple fatality motor vehicle crash on the West Virginia Turnpike. Of the three firefighters injured, Fire Chief Timothy Walker remains in the hospital in critical condition; Firefighter Billy Hypes is in stable condition; and Firefighter Kyle Jenkins was released with minor injuries.

USFA LODD: Ohio Firefighter On-Duty Death Notification 03.16.2018


After attending an earlier fire department training meeting, Assistant Chief Rodney D. Baker, Jr., 47, responded to a grass fire at 2229hrs on March, 15, 2018. Assistant Chief Baker (Chief 51) was first on scene as the commanding officer for the incident and was back in service at 2336hrs. The following day, March 16, 2018, while attending a family event, Assistant Chief Baker became ill and collapsed at 1638hrs. Family members trained in CPR immediately came to Assistant Chief Baker’s aid while the Pioneer Fire Department and Williams County EMS were called to the scene. Assistant Chief Rodney D. Baker, Jr. was transported to Bryan hospital where, despite all efforts, he later passed away from a reported cardiac event.

USFA LODD: Two Pennsylvania Firefighters On-Duty Death Notification 03.22.2018

On March 22, 2018 Firefighter Ivan Flanscha, 50, and Firefighter Zachary Anthony, 29, were killed from a collapse at the scene of a multi-alarm fire that began the previous day. Fire crews were still on-scene putting out hot spots that were flaring up nearly twenty-four hours later. Firefighters Anthony and Flanscha were transported to the hospital where they succumbed to their injuries. Two other firefighters injured in the collapse are reported to be in stable condition with non-life threatening injuries. Prior to the fire breaking out, the 150 year old multi-story structure, known as the Weaver Organ and Piano building, was being renovated into apartments. The cause of the fire remains under investigation by authorities.

USFA LODD: New York Firefighter On-Duty Death Notification 03.22.2018

On March 22,2018 Firefighter Michael R. Davidson, 37, was killed and two other firefighters were seriously injured in a Harlem five-alarm fire that broke out in a former jazz club that was being used as a film set. Firefighter Davidson was operating the fire hose nozzle in the basement of the building when fire conditions intensified and Davidson became separated from the other firefighters. Firefighter Davidson was removed from the structure and taken to Harlem hospital where he was declared deceased. The cause of the fire remains under investigation by authorities.

USFA LODD: Louisiana Firefighter On-Duty Death Notification 03.16.2018

While on-duty at the fire station, Firefighter-Operator Allen Headley, 31, was found unresponsive in the fire station day room. Firefighter-Operator Headly was treated by fellow responders and transported to the hospital but succumbed to a nature and cause of fatal injury still to be determined.

USFA LODD: Texas Firefighter On-Duty Death Notification 03.10.2018

Firefighter M.V. Hudson, 45, was injured in a fire tender (tanker) crash on the evening of February 28th.  Hudson and two other firefighters were responding to a grass fire when the apparatus left the right side of the roadway and rolled over, badly damaging the cab and injuring all three occupants. The three firefighters had to be extracted from the vehicle and were rushed to the hospital. Two firefighters were subsequently released, but Firefighter Hudson died while in the hospital on March 10, 2018.

USFA LODD: Georgia Firefighter On-Duty Death Notification 03.12.2018

On March 12, 2018 Firefighter Stacey Leigh Boulware, 44, collapsed while on duty at Cobb County Fire Station 5.  Firefighter Boulware was reported to having responded to two emergency calls and passing a fire department physical agility test in the hours prior to her falling ill. Firefighter Boulware was treated by fellow responders then transported to Kennestone Hospital where, despite all efforts, she succumbed to a nature and cause of fatal injury still to be determined.

Our condolences to the Boulware Family & Friends, Cobb County Fire and Emergency Services, and to all affected by Firefighter Stacey Leigh Boulware's passing.

USFA LODD: Vermont Firefighter On-Duty Death Notification 02.28.2018

On February 27, at 1526hrs, Lieutenant Casey Stephen Kuhns responded to a motor vehicle crash.  Later that day, Lieutenant Kuhns participated in fire department training that ended at approximately 2130hrs.  At 0600hrs on February 28, Lieutenant Kuhns was found deceased at home from a nature and cause of fatal injury still to be reported.

Our condolences to the Kuhns Family & Friends, Morrisville Fire Department, and to all affected by Lieutenant Casey Stephen Kuhns’ passing.
