USFA LODD: Kansas Assistant Fire Chief On-Duty Death Notification 02.14.2016

Assistant Fire Chief Daniel F. Cool, 71,  responded to an emergency incident on the morning of February 11, 2016 at 2020hrs.  Later that day, he attended a training meeting and collapsed suddenly from a heart attack.  Chief Cool was admitted to St. Francis Hospital in Topeka, KS, where he succumbed to his injury early in the morning of February 14, 2016.

USFA LODD: Kentucky Fire Lieutenant On-Duty Death Notification 02.10.2016

Fire Lieutenant David W. Conley, 48, on February 10, 2016 at 2145hrs, fell ill from an apparent heart attack while on-duty at the fire station. Lieutenant  Conley was treated by fellow responders and transported to the hospital in Morehead, KY, where he succumbed to his injury.

Fire Police Officer Louis “Pop Pop” Patti, 67, passed away on December 22, 2015, from a CVA injury suffered minutes after returning home from a response call on December 6, 2015.

Fire Captain Willie B. Ratliff, 66, on December 21, 2015 at 2115hrs, was found deceased in the firehouse by fellow firefighters from an apparent heart attack shortly after participating in fire department physical training at the fire station.

USFA LODD: North Carolina Firefighter On-Duty Death Notification 12.15.2015

Firefighter Sidney “Sid” Winer, 92, was involved in a motor vehicle collision on October 20th, 2015, while riding in department's Ford F150 returning from picking up equipment.  Another vehicle pulled out directly in front of the department's truck. Firefighter Winer sustained serious fractures to both feet as a result of the collision which totaled the department's truck.  As a result of the injuries he sustained, Firefighter Winer's right leg was amputated just below the knee on December 3rd, 2015.  Firefighter Winer was in the rehabilitation center at CarolinaEast Medical Center when he succumbed to his injuries on December, 15th.

USFA LODD: Texas Firefighter On-Duty Death Notification 12.19.2015

While involved in training exercises at the fire station, Firefighter Stacy A. Crawford, 47, on December 19, 2015 at at 1130hrs, suddenly became unconscious and collapsed. Resuscitation efforts were begun by fellow responders and Firefighter Crawford was transported to the hospital where he succumbed to a nature and cause of fatal injury still to be determined.

USFA LODD: New York Fire Captain On-Duty Death Notification 12.15.2015

Captain Jack H. Rose, 19, on December 15, 2015 at 1859hrs, responded with members of his fire department to a mutual aid response call for a reported chimney fire. According to the fire department, when firefighters arrived on scene they observed fire coming from the ground floor. An entry team of firefighters from the Mt. Marion Fire Department entered the basement to extinguish the fire. During the interior firefighting operation, Captain Rose became separated from his team. Rose was quickly located by fellow firefighters and removed from the basement. Once outside, Captain  Rose became unresponsive and firefighters initiated lifesaving measures. Captain Rose was treated by paramedics at the scene and transported by DIAZ Ambulance to the Health Alliance Hospital-Kingston Broadway Campus where efforts to revive Jack H. Rose  continued. In spite of all life saving measures, Captain Captain Jack H. Rose eventually succumbed to his injuries.

USFA LODD: California Fire Captain On-Duty Death Notification 11.30.2015

Fire Captain Scott Carroll, 48, on November 28, 2015 fell ill while working a duty shift that had included one cooking fire and two EMS calls. Captain Carroll left the station for his personal physician’s office and then home to rest where, two days later, on November 30, 2015, he was found by his wife having suffered a fatal brain aneurysm.

USFA LODD: Illinois Firefighter/Paramedic On-Duty Death Notification 12.04.2015

Firefighter/Paramedic Mark Zielinski, 49, on December 4, 2015 at 1445hrs, responded to a medical incident for a male who was in cardiac arrest at a local retail store. While performing CPR on the patient, Firefighter/Paramedic Zielinski suffered a medical emergency. A second ambulance was summoned and Firefighter/Paramedic Mark Zielinski was taken to Franciscan St. James Hospital in Olympia Fields where, despite all efforts, Zielinski passed away from a reported cardiac related injury.

Fire Captain Zach Chase Clevenger, 30, worked a structure fire in the evening of December 4th returning home near midnight. Firefighter Clevenger was found unconscious by his wife in the morning and emergency assistance was summoned. All efforts to revive Fire Captain Zach Chase Clevenger were unsuccessful and he passed away from a nature and cause of fatal injury still to be reported.

USFA LODD: Kentucky Assistant Fire Chief On-Duty Death Notification 11.24.2015

On November 12, 2015 at 2200hrs, while responding to a fire call, Assistant Fire Chief Terry “TC” Culver, 65, became ill with heart attack like symptoms causing him to fall from a piece of fire apparatus onto a concrete floor. Assistant Fire Chief Culver was treated and transported to the hospital but succumbed to his injuries almost two weeks later on November 24, 2015. The nature and cause of fatal injury is still to be reported.

On October 14, 2015 at 0816hrs, Fire Police Officer Gerald "Bear" Celecki, 70, collapsed while directing traffic at an emergency incident. Fire Police Officer Celecki, who was also chief of the Middlesex County Fire P, was treated on-scene by fellow responders and transported to the hospital but succumbed to a nature and cause of fatal injury to be determined

On October 13, 2015 at 0720hrs., Fire Captain Charles "Chuck" Horning, 54, was found deceased, slumped over the steering wheel of a vehicle, approximately eight hours after responding to a fire call with his department. The nature and cause of the fatal injury is still to be determined.

On October 6, 2015 at 1900hrs., Fire Lieutenant Antonio Smith, 46, fewer then 24 hours after responding to an emergency call with the Memphis Fire Department, was found deceased near his residence inside his personal vehicle. The nature and cause of Lieutenant Smith's fatal injury are still to be reported.

Fatal Fire At Villa Serene Apartments; Up-State New York 12.03.2014

ELMIRA HEIGHTS, N.Y.
Firefighters responded to the Villa Serene Apartment Complex at 399 East 14th St. at 4:48 a.m. Wednesday, where a blaze broke out on the 11th-floor. Multiple fire departments responded to the scene.
  
Fire officials have confirmed one death; Bernard G. Marshall, 63, died after falling from his apartment window that was on the 11th floor of the 12 story building.

 

 

"The fire appears to have started and was mostly contained to apartment 1103, which was the apartment of Marshall," according to Elmira Heights Police Chief Rick Churches said in a news release. "Marshall did fall from his apartment window, which was on the 11th floor of the 12-story building. Although cause of death cannot be stated at this point Marshall did appear to be escaping the intense flames and smoke that was coming from the apartment."

 

"The building is built in a way that contained it [the fire]," said Elmira Heights Fire Chief Michael Cadek. "The fire had a really good head start because it was already blowing out the window when we got the call."

 

The fire was under control by 5:15 a.m. Firefighters used a special technique to combat the flames

 

"We have an aerial device that we used to hit it [the fire] from the outside," said Fire Chief Cadek. "And then once we hit the majority of it, the crews went inside, because they had to climb up 11 floors to get there, then they made the initial attack from the inside."

 

120 people were evacuated from the building. Some units sustained water damage from the fire response. While some people were able to move back into the building by Wednesday evening, others will be out for a few days.

 

The Red Cross is assisting those displaced. Residents are staying optimistic. 

  

Chemung County Fire Investigators, New York State Fire Investigators and Elmira Heights Police are investigating the cause of the fire.

Two Buffalo, New York Fires 05.12.2012

Buffalo Firefighters responded early this morning at 0031 hrs for a report of a structure fire on Walden Ave. . Fire was showing from the rear of a two story vacant dwelling. There was reported fire on the first  floor with possible extension to the second floor and attic. It was by  further reported by firefighting crews the building structurally compromised and ordered defensive operations.




Buffalo Firefighters also responded to a fatal blaze at 2972 Bailey early this morning at 0055 hrs for a report of a structure fire on Bailey Ave.. The building is a mixed-use commercial and residential building.

The victim, John Ciszek, 59 years old, was unable to make it out lived in a rear apartment of the two-story building at 2972 Bailey,  and was pronounced dead on the scene. Mr John Ciszek's roommate Joseph Balsano said,"I heard the smoke alarm going off. That's what woke me up," then Balsano attempted to get access to the victim  but failed, escaped.

Buffalo Fire Department Division Chief Patrick Britzzalaro described "heavy fire conditions" in the first floor rear apartment where the two roommates lived.



Firefighting crews arrived with fire showing from a 1 1/2 story mixed use building and a report of a victim inside. There was reported heavy fire on the first floor of the occupied building with an exposure problem  and a requested for additional firefighting crews to the fire scene. The fire was under control in 36 minutes.

Pennsylvania Rowhome Fire, Three Children, One Adult Killed 3.27.2012

A vigil is planned for Wednesday night for the woman and three children who died Tuesday in a fire in Schuylkill County.

It comes as investigators search for the cause of the deadly blaze.

SHENANDOAH, Pa. - A fire destroyed five row homes in northeastern Pennsylvania early Tuesday, killing a woman and three children in one house while a fourth child managed to escape out a second floor window as the flames closed in, fire officials said.

The blaze spread quickly, and the buildings were fully engulfed when crews arrived within minutes of the 6:30 a.m. call. Firefighters attempted a rescue, but they made it only a short distance inside the front door of the victims' home before having to retreat.

"The fire was too intense," said Shenandoah Fire Chief Richard Mensick.

Four people, three of them children, died early Tuesday morning when fire raced through five row homes in Shenandoah. The dead included a 28-year-old woman, two ten year olds and a baby.

As the sad news rapidly spread through Shenandoah, the offerings of balloons, candles and flowers grew on East Coal Street, as people came to grieve for the young mother and three children killed in an early morning fire.

Schuylkill County Coroner Dr. David J. Moylan said Matejick Snyder and the three children all appeared to have died of smoke inhalation and ruled the manner of death accidental due to the fire.

State police fire marshal will be investigating and determine how the fire started. Cause of the fire is being labeled "undetermined" while the investigation continues.
